User Feedback Form

We need you to complete this form before we can release funds to the facility to pay for the access provided. Your completed form will be shared with the NNUF Management Group. We may also share parts of the form with the facility – please indicate if you would like any parts of your feedback to be kept confidential from the facility, or to be shared anonymously.

Before completing the user feedback form please ensure that you, and any team members whose details you may enter into the form, have read the privacy notice:

(It must be the Principal Investigator listed on the application form for the experiment, who completes this user feedback form on behalf of themselves and their research team.)
(as specified on your application form - plus the reference number for the experiment e.g. "NNUF Experiment 1". The reference number would have been included in the email from NNUF, informing you that your application had been approved).
